Skilled Nurse Visits
This program allows for TCPS Registered Nurses (RN) and Licensed Practical Nurses (LPN) to enter our client’s home, assess him or her, and provide cares ordered by their physician. Such cares include, but are not limited to:
Comprehensive assessment - including a total head to toe assessment of the child’s vital signs, growth and development, general well-being, and a review of medical concerns (diagnosis, medications, interventions, etc.).
Nutritional screenings
Weight checks
Oxygen management
IV Management
Injections (including Synagis)
Parental education, as needed.
Family centered support as needed.
Case Management and care coordination between all health care professionals involved in the client’s care.
