Tender Care Pediatric Services (TCPS) is committed to providing comprehensive and compassionate home health care for children with medical needs while supporting their families.  Our mission is to help each child and family understand and embrace their personal medical needs and incorporate the necessary cares to live an active, fulfilling life. We recognize the individual circumstances that are unique to each child, and we strive to assist our clients and their families to achieve the goals they have set for themselves.


Services offered:

In-Home Support Services (IHSS):

This program gives families the ability to direct and manage their care with the support of a trusted agency. Families can select, train, and oversee attendants who provide personalized services, including personal care, homemaking, and health maintenance.

IHSS offers assistance with essential tasks such as hygiene, medical management, mobility assistance, and respiratory care, ensuring a comprehensive approach to individualized care.

 

CNA Parent / Child Partnership:

This program allows parents of children with special needs to provide care to their children at home, after they have obtained their Certified Nursing Assistant Certificate. Such cares revolve around assistance with activities of daily living and include, but are not limited to:

  • Personal care – Assistance with bathing / Showering, Oral hygiene, Shaving, Perineal care, nail care, clothing changes

  • Skin care – Skin Care, GB / JB / Trach / Ostomy site cares

  • Nutrition – Meal preparation and feeding assistance

  • Elimination – Assistance with toileting, Catheter care, and Ostomy site cares.

  • Activity – Assistance with ambulation

 

Private Duty Nursing:

This program allows a TCPS RN / LPN to provide skilled care in the home for a child with medical needs for extended periods of time.

If additional services are needed outside of those that TCPS can provide, our RN Case Managers are always able and willing to assist the child / family in making referrals to the appropriate agencies.

Each TCPS client is partnered with a RN Case Manager. Case Managers have extensive experience and training in the pediatric field, as well as a thorough understanding of family interactions and medical concerns in the home setting. Our Case Managers will work closely with each child and their family in order to gain a complete understanding of their individual needs and goals. Next, an individualized, comprehensive plan of care is created to help each child and his or her family reach their goals. Frequent communication and case coordination between the client/family, Case Manager, and any other health care professionals involved is consistent,  to ensure that progress is being made towards the goals that have been identified.

Our nursing team is made up of very skilled Registered Nurses, Licensed Practical Nurses, Certified Nursing Aides, Occupational Therapists and Physical Therapists. Each member of the team has not only received the education and training necessary for their individual licenses and certificates, they have also received additional education and experience in the pediatric realm. Our staff has a passion for working in the pediatric medical field that is fueled by personal experiences. We want to help improve the world, one child / family at a time.

 

Skilled Nursing Visits:

This program allows for TCPS Registered Nurses (RN) and Licensed Practical Nurses (LPN) to enter our client’s home, assess them, and provide cares ordered by their physician. Such cares include, but are not limited to:

  • Comprehensive assessment - including a total head to toe assessment of the child’s vital signs, growth and development, general well-being, and a review of medical concerns (diagnosis, medications, interventions, etc.).

  • Nutritional screenings

  • Weight checks

  • Oxygen management

  • IV Management

  • Injections (including Synagis)

  • Parental education, as needed.

  • Family centered support as needed.

  • Case Management and care coordination between all health care professionals involved in the client’s care.
