Report illness and/or exposure to COVID-19

Contact your patient’s Case Manager if you observe signs/symptoms of infection in your patient during visits.

Contact the office to report signs/symptoms of infection in yourself, if you have traveled outside of Colorado, or if you think you have been exposed to someone who has COVID-19.

Watch for symptoms:

People with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness.Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:

  • Fever or chills

  • Cough

  • Shortness of breath or difficulty breathing

  • Fatigue

  • Muscle or body aches

  • Headache

  • New loss of taste or smell

  • Sore throat

  • Congestion or runny nose

  • Nausea or vomiting

  • Diarrhea

Temperature Monitoring Instructions

Complete temperature check upon arrival to your patient’s home and prior to patient contact.

1. Thoroughly clean/wash your hands.

2. Take Temporal Temperature. If over 99.8 F or 37.8 C repeat once. If second reading is above 99.8 F or 37.8 C, no patient contact, clean thermometer, leave home, and contact the office.

3. If temp is below 99.8 F or 37.8 C, clean thermometer with alcohol prep pad.

4. Document, “temperature check completed” in your SNAP narrative

COVID-19 Infection Prevention and Control

  • Practice standard and transmission based precautions.

  • Clean your hands often.

  • Refrain from touching your face.

  • Stay home if you are sick.

  • Nurses - complete temperature checks when arriving at PDN homes.

  • Cover coughs and sneezes.

  • Practicing social distancing, limit your interactions with others, and limit your travel.

  • Clean AND disinfect frequently touched surfaces daily. This includes tables, doorknobs, light switches, counter tops, handles, desks, phones, keyboards, toilets, faucets, and sinks.
